To start, download this resource to create your own editable spreadsheet by selecting 'File' > 'Download' > 'Microsoft Excel (.xlsx)' in the top left menu.

How to use this template:

1. Once downloaded, navigate to the tabs below titled 'Child', 'Parent' and 'Family' to start adding relevant information.

2. Add additional rows and columns as needed. Individually or as part of a team, identify what information (first column) and services (second column) you currently refer to.

3. Identify any gaps in information or services, using the prompts provided. If this list looks overwhelming, select five priority areas under each tab e.g. Child, parent, family.

4. Research services in your local community and add them to the relevant tabs. You can find research strategies in the Emerging Minds Referral Tip Sheet.

5. Your final document could be saved on your desktop/in a patient's file, laminated, dated, and stored/displayed for easy access or formatted as a personalised handout for patients.

6. Consider creating a referral pathway document for your practice team and review regularly (e.g. annually), as services may change depending on funding and government priorities.

This resource provides GPs with an opportunity to audit and map out new or changing services within the community.

It considers the needs of the child, parent and family and prompts you to reflect on how you can:

- better promote a child, parent or family’s engagement with social activities and groups
20
- create new connections with services to support children’s general health and development; and
21
- continue to foster positive emotional wellbeing for all family members.

The Pathways of Care can be used as a reflective exercise, organisational mapping/audit tool, or to help parents identify services they have previously used or are interested in using.
