 In order for us to establish classes and appoint staff, we require accurate information from our families in relation to your child/children’s enrolment at Dohertys Creek P-9 College next year.

Although we are disappointed to see anyone from the DCC family leave, we understand that family circumstances sometimes necessitate this. Therefore, could you please complete this short survey for EACH of your children by Friday the 21st of October.

** If your child is leaving Dohertys Creek P-9 College (we will be sad to see you go!), you must notify us via email to: dohertys.creek.p9.co@education.vic.gov.au

You must include the following information:
o Child’s name
o Family address and parent contact phone number
o Date of last attendance at Dohertys Creek P-9 College
o Reason for leaving
o New school name - this is very important as we are required to electronically transfer your child’s information to their new school

If you have another child/children that will be starting at Dohertys Creek p-9 College in 2022 and have not yet enrolled that child, please contact the office on 8560 6079 or visit our website at www.dohertyscreekcollege.vic.edu.au as soon as possible to download a digital version of our enrolment form that you can complete/sign electronically and return via email.
